MM4 asks for a new file when adding a new layer. How can I add a new blank live layer without using a file? The previous MM did this differently.

As in previous versions of Map Maker the live layer is always there. You simply clear the live layer from the Project Manager or else clear the current project from the file menu and you will have a blank live layer.
